When Is the Best Time to Test Hormones in Perimenopause?

The most common questions I hear are: “Can you test hormones in perimenopause?” and “When is the best time to test hormones in perimenopause?”
The answer depends on your menstrual cycle and in many cases, there isn’t a single “perfect” day but testing can be helpful at guiding treatment and giving us some insight into what is going on.
Why Hormone Testing in Perimenopause Can Be Challenging
During perimenopause, estrogen and progesterone levels naturally fluctuate from day to day and cycle to cycle. Unlike the predictable hormone patterns seen in younger women, these changes can make interpreting a single hormone test more difficult.
That’s why hormone testing should always be interpreted alongside your symptoms, menstrual history, and overall health. We don’t rely solely on the numbers.
Best Time to Test Hormones in perimenopause If You Still Have Regular Periods
If your cycles are still relatively regular, the ideal time to test depends on which hormones you’re evaluating:
- FSH, LH, and Estradiol: Typically tested on cycle days 3-5 to assess ovarian function.
- Progesterone: Best measured about 5-7 days after ovulation (often around day 21 of a 28-day cycle) to determine whether ovulation occurred and how robust your progesterone rise is.
What If Your Periods Are Irregular?
Hormone testing in perimenopause can become tricky. As cycles become irregular, timing hormone testing becomes less important. In these cases, we may order testing regardless of where you are in your cycle and focus more on trends and the overall clinical picture rather than a single number and oftentimes symptoms are more important with irregular cycles than the numbers.
Some tools can be helpful like ovulation predictor strips or LH strips to see if ovulation is occurring. This can be helpful with irregular cycles or if patients have and IUD or have had a hysterectomy.
Which Hormones Should Be Tested?
Depending on your symptoms, we may recommend evaluating:
- Estradiol
- Progesterone
- FSH and LH
- Thyroid function (TSH, Free T4, Free T3)
- Testosterone and DHEA-S
- Vitamin D, ferritin, and other nutrient levels that can contribute to fatigue or hair loss to rule non-hormonal causes.
