Why “Normal” Lab Results Don’t Always Mean Optimal Health (and what optimal lab ranges are)
So you’ve been told by your PCP that your labs are “normal”, yet you still feel exhausted, moody, bloated, or stuck in a cycle of unexplained symptoms. This is one of the most common frustrations I hear from new patients come to Well North Functional Medicine. It highlights an important truth: normal lab results do not always equal optimal health, we should be aiming for optimal lab ranges if we want to achieve optimal health. Makes sense, right?
The Problem with “Normal” Lab Ranges
Conventional lab ranges are designed to identify disease, not wellness. These ranges are created by averaging values from a large population (aka our not so health US population), many of whom are already dealing with chronic stress, inflammation, poor sleep, and nutrient deficiencies. As long as your results fall within that wide statistical range, they’re often labeled “normal,” even if your body isn’t functioning at its best.
In other words, normal means you don’t have a diagnosable disease but that doesn’t mean your body is thriving.
What Are Optimal Lab Ranges?
In functional medicine, we look at optimal lab ranges, looking at narrower, evidence-informed targets associated with better energy, hormone balance, metabolic health, and resilience over time. These optimal lab ranges are based on clinical outcomes, not just population averages.
A few examples:
- A “normal” thyroid marker (TSH) may still be associated with fatigue, hair loss, or weight gain. We must take a look at the whole thyroid panel and at a narrow range where most people feel optimal.
- Ferritin (our iron storage indicator) can be technically normal while still too low to support energy, exercise and sleep. We are actually starting to see professional societies look at more optimal lab ranges for ferritin, like we have in functional medicine for a long time. Check the American Society of Hematology’s 2023 position statement HERE! This is very encouraging because I see iron deficiency in menstruating women ALL THE TIME!
- Your fasting blood sugar or A1c may be normal, yet fluctuate enough to drive cravings, anxiety, and poor sleep or maybe it is creeping up toward the prediabetes range. Health and disease are a spectrum and we don’t want to to be normal but 1 point or .1 away from getting labeled with a disease!
Striving for optimal lab ranges means identifying early imbalances before they become full blown disease.
Why Symptoms Matter More Than a Lab Flag
Your body doesn’t read lab reports, it responds to physiology and is often telling you something! Symptoms are often the earliest signal that something is off, even when labs haven’t crossed a disease threshold yet.
