HRT Safety: The Estrogen Black Box Warning Has Been Removed — Here’s What That Means for HRT
For decades, many women avoided hormone replacement therapy (HRT) because of the black box warning placed on estrogen products after the 2002 Women’s Health Initiative (WHI) study. That warning shaped an entire generation’s perception of menopause care and HRT safety.
Now, recently updated labeling reflects what we’ve known for years in functional medicine: estrogen therapy, when prescribed appropriately, is safe and effective for most healthy women in menopause.

Why Was Estrogen Given a Black Box Warning in the first place?
The original warning, placed on all estrogen products, was based on early data from the WHI study, which raised concerns about breast cancer, heart disease, stroke, and blood clots in women taking estrogen.

- The average participant was 63 — well past menopause
- Most were more than 10 years from their last period
- Oral conjugated equine estrogen was studied (not modern bioidentical estradiol)
- Risks vary significantly based on age, timing, and route of delivery

For women under 60 or within 10 years of menopause, the risk profile looks very different. In addition, the formulations of estrogen that are used today are vastly different than the estrogens used in the WHI. We’ve come a long way and this all matters, significantly.

HRT and HRT safety is not a one size fits all model.
