When Is the Best Time to Start HRT?
One of the most common questions I hear at Well North Functional Medicine is:
“When is the best time to start HRT?”
“Am I too early… or already too late for hormone replacement therapy (HRT)?”
The short answer: The ideal window is wider than most women think and starting at the right time can dramatically change your perimenopausal experience.
The Gold-Standard Timing: Early Menopause Transition (or Even Perimenopause)
Research (including the 2022 NAMS guidelines and the landmark SWAN study) shows that it is safest and patients have the best outcomes occur when HRT is started:
- Within 10 years of your final period, OR
- Before age 60
Women who begin in what we call the “window of opportunity” see:
- 50–70% reduction in hot flashes and night sweats
- Up to 50% lower risk of heart disease and osteoporosis
- Better brain protection (lower risk of cognitive decline)
- Dramatically improved mood, sleep, and libido
Starting after age 60 or more than 10–15 years post-menopause still helps symptoms and may be safe for many women, but some of the long-term cardiovascular and bone benefits are reduced and at this point we do have to keep in mind other cardiovascular risk factors. So the best time to start HRT is as soon as you have symptoms!
What Most Doctors Miss: You Don’t Have to Wait Until Full Menopause
You do not need to be in full-blown menopause or post-menopausal to benefit. I work with many perimenopausal women in their late 30s and early 40s that are starting to have symptoms of hormone decline who benefit greatly from starting HRT early.
Early intervention can:
- Help with early symptoms of perimenopause – difficulty sleeping, anxiety, hot flashes, low libido, body composition changes.
- Prevent the downward spiral of worsening symptoms
- Preserve collagen, vaginal tissue, and bone density
- Protect sleep and mood before they completely tank
Red Flags That Now is the Best Time to Start HRT
- Hot flashes or night sweats disrupting sleep
- Brain fog, anxiety, or new depression that started with cycle changes
- Painful sex or recurring UTIs due to vaginal dryness
- Rapid loss of bone density on a DEXA scan
- Family history of osteoporosis, heart disease, or Alzheimer’s
When to Pause and Investigate First (Functional Medicine Step)
Before jumping straight to HRT, we always rule out or correct:
- Uncontrolled blood sugar or insulin resistance
- Untreated hypothyroidism or Hashimoto’s
- Severe adrenal dysfunction / high cortisol
- Significant nutrient depletions (especially magnesium, D, B6, B12)
- Gut dysbiosis or estrogen-reabsorbing “estrobolome” issues
Fixing these first often means you’ll need lower doses or get dialed on the right dose of HRT more quickly— and you’ll feel the benefits faster.

The “best” time to start HRT is when symptoms are impacting your quality of life and you’re still within roughly 10 years of your final period. For many women, that sweet spot is in the early to mid 40s to mid-50s — exactly when perimenopause hits hardest, but don’t count it out if you are in your mid to late 30s either!
