Why Female Athletes Struggle With Hormone Balance

Many female athletes appear healthy on the outside, yet struggle with hormone imbalance behind the scenes. Intense training schedules, inadequate fueling, and insufficient recovery can place chronic stress on the endocrine system disrupting key hormones that regulate the menstrual cycle, metabolism, and performance. Low Energy Availability and Hormone Imbalance in Female Athletes

One of the most common causes of hormone imbalance in female athletes is low energy availability—when calorie intake does not meet the demands of training. Even mild under-fueling can signal the body to conserve energy, leading to low estrogen, low progesterone, irregular periods, or missed periods.

This pattern is often part of something we call RED-S (Relative Energy Deficiency in Sport), a condition that affects reproductive hormones, bone density, thyroid function, and overall athletic performance.

Training Stress, Cortisol, and Female Hormones

High training loads increase cortisol, the body’s primary stress hormone. Chronically elevated cortisol can suppress ovulation and interfere with estrogen and progesterone production, contributing to menstrual cycle irregularities, fertility challenges, and poor recovery. Over time, hormone disruption in female athletes may also affect:

      • Thyroid hormones and metabolism
      • Sleep quality
      • Mood and anxiety levels
      • Injury risk and bone health

Common Hormone Symptoms in Female Athletes

Hormone imbalance doesn’t always show up clearly on standard labs. Common hormone-related symptoms in female athletes include:

      • Irregular or absent menstrual cycles
      • Worsening PMS or new-onset PMS
      • Low libido
      • Fatigue and poor workout recovery
      • Sleep disturbances
      • Anxiety or mood changes
      • Hair thinning or hair loss
      • Cold intolerance or sluggish metabolism
      • Stress fractures or recurrent injuries

These symptoms are often dismissed as “normal for athletes,” or athletes are told they should simply “do less” (aka – stop running or biking or whatever their sport is). This is not helpful. These symptoms are important signals the body may be under-fueled or over-stressed.
