Why Hormone Replacement Therapy (HRT) Alone Isn’t Enough
Why Hormone Replacement Therapy (HRT) Alone Isn’t Enough – The Functional Medicine Difference
If you’re in your late 30s and beyond and struggling with hot flashes, brain fog, low energy, body composition changes or mood swings, Hormone Replacement Therapy (HRT) is sometimes presented as the quick fix. And yes, HRT can be life-changing when done correctly. Hormone replacement therapy restores declining estrogen, progesterone, or testosterone levels and can dramatically reduce perimenopausal and menopausal symptoms.
But here’s what most clinics don’t tell you: HRT alone treats the numbers on a lab sheet and is only looking at one body system! That’s where functional medicine changes everything.
Why Hormone Replacement Therapy Falls Short Without a Deeper Approach
- It doesn’t fix the upstream triggers
Chronic stress, poor sleep, blood-sugar imbalances, toxin exposure, and gut dysfunction all drive hormone disruption. If cortisol is sky-high from chronic stress, simply adding estrogen or progesterone is like pouring water into a leaky bucket. If you’re only sleeping 5 hours a night, your testosterone is of course going to be tanked and while hormone replacement therapy might help, we need to address all of the pieces to create truly optimal health. - It ignores nutrient deficiencies
I see nutrient deficiencies commonly in my practice, even in patients who eat pretty well balanced diets! Magnesium, vitamin D, B12, zinc, and omega-3s are required co-factors for hormone synthesis, receptor sensitivity, and detoxification. Many women start HRT while still deficient in many of these nutrients which can limit results and increasing side effects. I like to get a full comprehensive lab panel BEFORE we start HRT so that we can work on any deficiencies and optimize everything, not just hormones. - It overlooks inflammation
Excess inflammation from our modern world including toxins, gut dysbiosis or increased intestinal permeability, stress, blood sugar rollercoaster and more can impact how well we handle hormone replacement therapy. This is one reason some women feel worse on HRT even when doses look “perfect.” When we address every body system and reduce sources of inflammation, hormone replacement therapy is like the cherry on top! - It rarely addresses insulin resistance or thyroid dysfunction
Up to 50% of perimenopausal women have underlying insulin resistance or subclinical hypothyroidism – both of which sabotage hormone balance regardless of how much HRT you take. Our hormones work together in a symphony and if our blood sugar or thyroid are not on point, we will be totally out of tune.

How Functional Medicine + HRT Becomes a True Game-Changer
At Well North Functional Medicine, I never view HRT as a stand-alone therapy. Instead, I use it as one powerful tool inside a comprehensive root-cause plan that is personalized to you!
- Cycle timed hormone testing including: estradiol, progesterone, testosterone, free testosterone, sex hormone binding globulin, cortisol and DHEA.
- Nutrition and lifestyle protocols to lower inflammation and balance blood sugar with continuous glucose monitoring for even more personalization.
- In depth micronutrient testing and targeted supplementation to support all of our bodies cellular processes.
- Gut restoration and microbiome optimization (because 90% of estrogen is cleared through the gut) and bloating and constipation are NOT normal!
- Stress and sleep interventions to calm the HPA axis and allow our bodies the necessary time to recover.
- Ongoing monitoring, check-ins and adjustments so you feel amazing – not just “normal.” (One patient literally emailed this morning saying: “Thank you for always checking in. It’s a far cry from normal doctors office. I appreciate it!”)
When you correct the foundation first (or alongside) HRT, patients typically report:
- Faster symptom relief
- Lower and more dialed HRT doses
- Fewer side effects
- Sustained results even years down the road (because our health isn’t just about how we feel now but also our later decades!)
Hormone replacement therapy can be wonderful, but it works best when your body is primed to receive and use those hormones optimally. Functional medicine doesn’t compete with HRT, it completes it!
